International Travel Request for Deans and Faculty

International travel must be approved by the dean of your school prior to making any financial commitments to the trip. If this form is submitted by a faculty member, a follow-up e-mail will be sent from Academic Affairs to the dean of your respective school for approval. There is a two working day turn around (after the dean has approved the request) when seeking approval from Academic Affairs for International Travel. Please plan accordingly. It's not necessary to fill out this form for personal travel. Once approved, this form will be forwarded to Human Resources for travel insurance purposes.
